General information about this form:

Side this NOTES section is set to not visible, so it is not part of the form and is not transmitted to the user.

Special Fields

We are using the Contact's POSITION for the grade level 3 to 12

For the Group Contact/Parents the POSITION will hold the value of GROUP CONTACT

There are special styles and scripts in use.

Check the DETAILS > SEO tab to see what is used.

The first style hides the little Group registration radio button since it is always enabled by default by EA.  However we do use the label part of that radio button element to pass information to the user

The second and third items  automatically checks the "I'm not attending the event but am the group contact" check box.  We want every registration to have a parent as the group contact person.  we DO NOT want the first registrant to be the group contact as that would be a child. So we always want the "I'm not attending the event bu am the group contact" check box checked and we don;t want the user to be able to change that.

so the  style /* Hide the I am not attending but just registering people as group contact check box  */  hides the box so it cant be changes and the

/*Default Check I am not attending Group contact info */ script prechecks  so that that group contact details appear.

Step 3: Child's (registrant) Information

Child Information

Please enter child's personal information in the appropriate spaces below.

Please provide the Parent/Guardian Email Address.

Please list any allergies you would like our instructors to be aware of in the space provided. If your child has no allergies, you may leave this section blank.

Please note that camp staff are not able to administer medication.

Special Accommodations / Additional Information

Please share any information you feel would be helpful for our instructors to know, or let us know if your child requires accommodations (such as physical, social/emotional, gifted & talented, ADHD, learning differences, etc.) to fully participate in the camp. If no accommodations are needed, you may leave this section blank.

Engineering Design Summer Camps

Objective:

The Engineering Design Summer Camp, offered through the K-12 Student Outreach Program at Colorado School of Mines, provides an opportunity for high school students to attend a 5-day summer camp on the Mines campus in Golden, Colorado. The objective of the camp is to guide participants through a fun and rewarding hands-on experience of authentic engineering design practices. This camp will introduce a tool to build and make parametric models of parts and assemblies. Learn with a computer-based tool, through a software program called SolidWorks™ by Dassault Systems, which allows for creative modelling and visualization. SolidWorks™ is an integral part of engineering design and/or problem solving.

Prerequisites

Students must be entering in the fall of 2025 one of the grade levels shown for a camp in order to participate. Please be sure to check the grade levels associated with each camp, since not all camps are available for all grade levels. Sessions I, II, III and IV are the same camp, but offered multiple weeks.

Sessions I, II, III and IV are the same camp, but offered multiple weeks, so only one session can be selected. Each session is limited to 25 students.
