Welcome Conference and Event Services' Online Registration for K-12 Youth Camps

You can use this form to register either a single child or a group up to five (5). Please be sure to read the information below explaining the process for registering a group (more than one) child.  Please visit learn.mines.edu/k-12 for individual program descriptions and age groups.

Colorado School of Mines General Release and Liability Waiver

I wish to have my child (hereinafter the “Participant” or "Child") participate in the K12 Outreach Program(s) (hereinafter the “Activity”) hosted by Colorado School of Mines between June and August 2025.

I understand that the Activity will be conducted for the entertainment and educational benefit of the participating children and acknowledge that my Child’s participation in the Activity is voluntary. In consideration of Colorado School of Mines (“Mines”) making arrangements for and permitting my Child to take part in the Activity, I, the undersigned Parent or Legal Guardian, agree to hold harmless, release, indemnify and forever discharge Colorado School of Mines, and its Board of Trustees, officers, directors, employees, agents, and any persons acting on their behalf, as well as their heirs, executors and assigns, from and against any and all liability, claims, demands, costs and expenses (including attorneys’ fees) arising out of or in any way connected with any bodily injury or property damage in any way relating to or arising out of my Child’s participation in the Activity, including transportation to and from Activity events, even if the liability, claims, demands, costs and expenses may arise, in whole or in part, out of my Child’s use of equipment and facilities provided by Mines or the negligence or carelessness of the persons or entities mentioned above.

I am aware that the Activity, including transportation to and from Activity events, may include certain risks and dangers. I understand that specific risks vary depending on the level and nature of the Activity, and can include the risk of loss, property damage or personal injury. Personal Injury can range from minor personal injuries such as scratches, bruises, and sprains to major injuries such as eye injuries and back or joint injuries, to catastrophic injuries resulting in paralysis or death. By signing this waiver, I consent to my Child’s participation in the Activity and acknowledge that any and all risk, whether known or unknown, is expressly assumed by me and all claims, whether known or unknown, are expressly waived in advance.

To the best of my knowledge, I am not aware of any mental or physical disability or health-related reasons or problems that would hinder or otherwise prevent my Child from safely participating in the Activity or jeopardize the well being of others during the Activity.

I understand that I am solely responsible for any costs arising out of any bodily injury or property damage sustained through or arising from my Child’s involvement in the Activity. Further, I acknowledge that I am responsible for all my Child’s medical expenses. By acknowledging this waiver, I give permission in cases of my Child’s injury or illness to render emergency first aid and to make any necessary referral for treatment.

I hereby certify that I have carefully read this Release and Liability Waiver Form, and that I understand the contents herein. Nothing in this waiver should be construed to waive, limit, or otherwise modify any governmental immunity available to any of the persons or entities released herein under the Colorado Governmental Immunity Act, §24-10- 101, et seq., C.R.S.

Colorado School of Mines Technology Waiver

The Colorado School of Mines Technology Waiver is composed of two parts:

1. Technology Resource Access Agreement for Affiliated Programs
2.  Information for Parents/Guardians Regarding Technology Resource Access

Technology Resource Access Agreement for Affiliated Programs

Introduction

The Colorado School of Mines ("Mines") provides technology resources, including but not limited to computer accounts and internet access, to participants of Mines or affiliated programs for the purpose of promoting academic excellence through resource sharing, innovation, and communication. Users of these resources are required to utilize them in an ethical and legal manner. Violation of the provisions outlined herein will result in termination of access. It is hereby acknowledged that all Mines technology resources are the property of the Colorado School of Mines, and users shall have no expectation of privacy in the use of these resources or the data stored within them.

Guidelines for Use

Mines technology resources are provided solely to support and enhance the academic experience of affiliates and program participants. Users are required to be present while utilizing public lab spaces, computer systems, or terminals. The following actions are strictly prohibited:

• Utilizing Mines technology resources for business or personal financial gain
• Intentionally causing disruption to computing operations, information systems, or networks
• Violating software licensing agreements or copyright laws
• Compromising system security or resource availability
• Infringing upon the privacy of other individuals
• Transmitting material that may be deemed offensive

Violations of these terms will result in the revocation of technology resource access and may lead to disciplinary or legal action.

Privileges and Responsibilities

The use of Mines technology resources is a privilege. Mines leadership and system administrators retain the authority to determine what constitutes inappropriate use and to execute final decisions regarding access termination. Users are expected to adhere to general rules of network use, including but not limited to:

• Refraining from abusive or offensive communications
• Avoiding the use of inappropriate language
• Protecting personal information and that of other Mines community members
• Refraining from actions that may be construed as illegal activity
• Preserving network availability for other users

Security Protocols

Users who identify security issues must immediately notify soc@mines.edu or call the Mines Service Center at 303-384-2345. Attempts to reproduce or demonstrate such issues to other users are prohibited. The following actions are strictly forbidden:

• Sharing access credentials
• Accessing accounts not explicitly assigned for use
• Any unauthorized access or access attempts to technology resources

Violations will result in access revocation and may lead to disciplinary or legal action. Users identified as security risks may be denied access to Mines resources.

Vandalism

Vandalism, including but not limited to physical harm to equipment, malicious data manipulation or destruction, and the creation or downloading of malware, will result in the revocation of technology resource access and may lead to disciplinary or legal action.

Penalties

Intentional, repeated, or egregious violations of these policies will result in the loss of resource access. Appropriate disciplinary or legal action may be taken as applicable.

Information for Parents/Guardians Regarding Mines Technology Resource Access

Technology resource access is provided to your child (hereinafter the “Student”) participating in affiliated programs to promote academic excellence. While the valuable information and interaction available through these resources far outweigh potential risks, parents and guardians should be aware that Mines does not provide content filtering for internet access.

The Student will have access to various resources, including but not limited to:

• Email, chat, social media, and other interactive communication platforms
• Worldwide websites, including those with adult themes
• Public domain and shareware software
• Discussion groups and global news sources
• University library content and other academic databases

Proper use of Mines technology resources depends on the responsible conduct of users. Parents and guardians are encouraged to emphasize to your Student the importance of ethical and legal use of these resources. Violation of the provisions outlined in Mines technology resource use agreements will result in the Student's immediate termination of access and may lead to disciplinary or legal action.

Acknowledgment and Agreement

By signing this document, I confirm that I have read, understood, and agree to the following:

I acknowledge my responsibilities regarding the use of Mines technology resources and sharing it with my Child. I further understand that failure to fulfill these responsibilities may result in restricted or terminated access for my Child. Furthermore, I accept financial responsibility for any damage to equipment or property at the Colorado School of Mines resulting from my Child's actions.

COLORADO SCHOOL OF MINES Photo / Multimedia Model Release

All photographers and videographers taking photographs, videos or audio recordings on university property or at university events in their capacity as Mines’ employees must obtain a signed release from any student, faculty member, staff member, or community member who is visibly recognizable in the photograph or recording prior to making any commercial use of such photograph (including use in all public relations materials and marketing materials used to promote or market Mines’ or its departments such as websites, newsletters, brochures, magazines, signage or other similar materials).

Crowd scenes where no single person is the dominant feature are exempt as are photographs taken of news events. However, photographs taken for news purposes require a release prior to reuse in public relations or marketing materials.

 I understand that participants in Mines events are sometimes photographed and videotaped for use in promotional and education materials. I understand that such audio, video, film and/or print images may be edited, duplicated, distributed, reproduced, broadcast, and/or reformatted any form and manner without payment of fees.

I authorize Mines to record and photograph my Child’s image for use by Mines or its assignees in research, educational and promotional programs.

I hereby grant Colorado School of Mines permission to use likeness of my Child in photograph(s)/video or audio recordings in perpetuity, in any and all of its publications and media (including the internet), now or in the future, controlled by Colorado School of Mines. I waive any right to inspect or approve the finished product, and I waive any right to compensation. I release, indemnify and hold harmless the Colorado School of Mines and its Board of Trustees, officers, directors, employees, agents and representatives from any claims, damages, or liability arising from or related to the use of the photograph(s)/video or audio recordings.

If you paid for your registration online using a credit or debit card...

A receipt for your payment has been sent to your email address. In subsequent correspondence you will receive further information about the camp(s) for which you registered.

If you selected to be on a wait list...

If an opening becomes available for the wait listed session, we will contact you by email to ascertain if you still wish to participate.  You will have 24 hours to respond, otherwise, we will give the opening to another student.
